I am currently taking a limited number of new clients, and online appointment booking may not be available at all times. Due to how full my schedule is running, open appointment times will likely be at least a month out. At present, masks are encouraged but not required (unless you are experiencing symptoms, such as recovering from a cold). First time clients:  I now have a digital intake form which should be completed from your own device before you come for your appointment.  I will also send you a link for a specific COVID-19 form (completed for every appointment).  If you prefer to print your form, you can click here to download the Intake Form to fill out and bring to your appointment.  This is a fillable Adobe form-- just fill in your information and print.  Or if you prefer, you can print and then fill it out.  Be sure to sign your form after you print it.  Again, this must be done before you come for your appointment.  At this time I prefer that you submit the digital form if possible.
